This is just a guess but when you attach the foot to the machine, where does your needle line up. On the left side of the toe??? That would be my assumption and then you sew with the edge of your quilt block aligning with the right side of the toe cause the toe is probably a 1/4 inch wide. But I've gotta admit, I can't see a use for that guide...

BTW: thought I'd do some googling...and look what I found...a site that has videos of the feet. They don't have anything about the guide but they mention about the markings on the quarter inch foot to help you pivot...cool. And take a look at the PDF file for the quarter inch...it explains how to use it.
